The St. Paul Athletic Department requires that all athletes travel to and from all athletic contests on the team bus. However, we do realize there maybe family emergencies and situations when an exception might need to be made. In this case, this permission form must be completed and handed into the coach AND athletic director at least 24 hours in advance of the game.

According to CT state law, as a requirement for entrance to Grade11, students are required to have a physical examination in either Grade 9 or 10. Juniors will not be allowed to attend school without a physical form on file. Students who wish to participate in sports must have a sports physical done yearly and is valid for 13 months. Athletes will not be allowed to practice unless a current physical examination form is on file. The Bristol/Burlington Board of Health physical form is enclosed and can also be downloaded from the St. Paul website under the athletics tab and the parents tab.
